Breathing Apparatus.Certificate.NZ Breathing apparatus training	The required evidence will be one of the following: A certificate or competency card demonstrating completion of a breathing apparatus training course, issued by an NZQA accredited training provider; OR NZQA Record of Achievement showing completion in the last year (12 months) of either of the following unit standards: 25044 OR 3272 Course completion date must be visible and current Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted). Copy must be clear and legible Can be in colour copy OR black and white copy	Issue Date= Course completion date Expiry Date= 1 year (12 months) from issue date
Hazardous substances.Course.Hazardous substances awareness	The required evidence will be one of the following: A training certificate or card issued by a NZ provider, demonstrating completion of a hazardous substances safety training (or equivalent title); OR NZQA Record of Achievement showing completion of unit standard 31292 Demonstrate knowledge of workplace procedures relating to hazardous substances; OR Higher level certificates (person in charge of handling and storing hazardous substances, certified handler) Course completion date must be visible and current Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted). Copy must be clear and legible Can be in colour copy OR black and white copy	Issue Date= Course completion date Expiry Date= No expiry date
Fire Fighting.Course.Fire safety and extinguisher training	The required evidence will be one of the following: Certificate or competency card issued from a NZQA accredited training provider showing completion of a 'Use of fire extinguishers and fire safety' or 'fire suppression methods' course, of 'Hot works and safety watch' NZQA Record of Achievement showing completion of unit standard 3271 in the last 2 years Course completion date must be visible and current Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted) Copy must be clear and legible Can be in colour copy OR black and white copy	Issue Date= Course completion date Expiry Date= 2 years from issue date
First Aid.Certificate.First Aid & CPR training	The required evidence will be one of the following: Certificate or competency card showing completion of a New Zealand first aid course; OR NZQA record of achievement mentioning one of the combination of unit standards 6401 AND 6402 OR 26552 AND 26551 achieved in the last 2 years Training provider identification (name) must be visible. Must be a NZQA accredited training provider Course completion date must be visible and current Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted). Copy must be clear and legible Can be in colour copy OR black and white copy	Issue Date= Course completion date Expiry Date= 2 years from issue date
