• The required evidence will be any of the following:
  • Certificate of competency or competency card showing completion of one of the following unit standard, as relevant to the type of crane operated:
    • 16617 - operate a truck loader crane
    • 20208 - operate a self-erecting tower crane
    • 3790 - operate a cab controlled gantry crane
    • 3800 - operate a radio remote or pendant controlled gantry crane
    • 24511 - configure and operate a non-slewing articulated crane
    • 26505 - configure and operate a mini-crawler crane
    • 26590 - hydraulic handi crane
    • 27676 - lattice boom track crawler crane
    • 3794 - tower crane
    • 3795 - mobile crane
  • NZ Certificate in Crane level 3 or 4
  • NZQA Record of Achievement showing completion of Crane level 3 or 4 OR any of the unit standards listed above if completed in the last 3 years

• Course completion date must be visible
• Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Copy must be clear and legible
• Can be in colour copy OR black and white copy

• Issue date= Course completion date
• Expiry date= 3 years from issue date

• The required evidence will be one of the following:
  • Certificate of competency or competency card showing completion of an OSH forklift operators’ course or Operations and instructions of powered industrial lift trucks (forklifts) OR Approved Code of Practice for Training Operator and Instructors of Powered Industrial Lift Trucks (Forklifts)
  • NZQA Record of Achievement showing completion of unit standard 10851 in the last 3 years

• Issued date or expiry date must be visible
• Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Copy must be clear and legible
• Can be in colour copy OR black and white copy

• Issue Date= Licence issue date on certificate
• Expiry Date=As per certificate (3 years from issue date) 

The required evidence will be one of the following:

• Certificate or competency card issued from a NZQA accredited training organisation showing completion of training against standard 30072 (must be visible on the certificate or combined with course transcript showing unit standard achieved), OR
• NZQA Record of Achievement showing completion of training against unit standard 30072
  
  
• Date of training completion or date of unit standards achieved must be visible
• Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Copy must be clear and legible
• Can be in colour copy OR black and white copy
• Evidence must be issued by NZQA Private Training Establishment
