• Applicants name to be shown on documentation

• Applicants name must match (However shortened versions such as Chris for Christopher can be accepted)

The following evidence can be accepted:

Site Authorisation Form: “RTAY Contractor Assessor Authorisation”

• All assessment pages within upload

• The form must clearly indicate the current TAE qualification held, which must be one of the following:

  • Certificate IV in Training and Assessment TAE40122

  • Certificate IV in Training and Assessment TAE40116

  • TAE40110 (including the unit TAELLN411 – Address adult language, literacy and numeracy skills)

  • An equivalent Diploma or higher-level qualification in Training and Assessment

• Candidates signature and signed date

• L&D Assessor's name, role, signature, and date

• Candidate Authorised must be ticked

• A copy of one of the following Statements of Attainment must be uploaded with the Contractor Assessor Authorisation Assessment:

  • Certificate IV in Training and Assessment TAE40122

  • Certificate IV in Training and Assessment TAE40116

  • TAE40110 (including the unit TAELLN411 – Address adult language, literacy and numeracy skills)

  • An equivalent Diploma or higher-level qualification in Training and Assessment

NOTE:

• The form will show "RTAY Contractor Assessor Authorisation"

Issue date = Date of training completion.

Expiry date = 2 years from issue date

Training & Assessment.Certificate IV.Trainer & Assessor

• Applicants name to be shown on documentation

• Applicants name must match (However shortened versions such as Chris for Christopher can be accepted)

• Evidence must be issued by an Australian Based Recognised Training Institution

• Actual copy of certificate / degree is required. Letters are not accepted in lieu of this.

• Completion date or issue date must be displayed

• The course title and qualification level must be clearly stated on the certificate and must match one of the following:

  • Certificate IV in Training and Assessment TAE40122

  • Certificate IV in Training and Assessment TAE40116

  • TAE40110 (including the unit TAELLN411 – Address adult language, literacy and numeracy skills)

  • An equivalent Diploma or higher-level qualification in Training and Assessment

• Document / Certificate / Number

• Issue Date = Date of Training completion

• Expiry date = No Expiry

RTA Managing Fatigue Awareness ;Course code 30637698, Qual code 30278486

• Applicants Legal name to be shown on document and must match cardholders registered name in Onsite

• RTAY Recognition of Prior Competency is acceptable

• Record of Completion certificate issued from Cornerstone / Rio Tinto College

• Must include all (3) assessment pages if completed face to face

• Document must list training number 30637698 or 30278486., Completion certificate has no codes to reference

• Black and white or colour is acceptable

• Scanned document to be clearly legible

• Issue Date to be entered as date of training completion

• Expiry date to be entered as 2 years from the issue date

  • Shortened versions of first names are accepted, such as:
    "Chris" instead of "Christopher"
    "Wil" instead of "William"

    "Mohd" instead of "Raza Hasan Mohd"

• The following forms of evidence can be accepted:

  • ILT attendance record

• Issue Date = Date of training completion
