Confined Space.Certificate.RTAY Confined Space Entry - CTO; 30772792, 30404390	Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable) Must be 'CTO' training. If 'CTO' is missing from the certification name it is a different training Both training numbers (30772792 and 30404390) must be listed on the document. If there are checkboxes next to them, the box must be checked All assessment pages must be attached and completed "RTAY Confined Space Aware and Standby CTO" is an acceptable replacement for this training Acceptable replacement for this training must include the following on Recognition of Prior Competency document: RTAY C5 Confined Space Awareness (30404390) RTAY Confined Space Entry - CTO (30772792) Assessor signature and date must be completed Black and white or color is acceptable Scanned document to be clearly legible	Issue Date to be entered as date of training completion Expiry date to be entered as 2 years from the issue date
