• Assessor's name, signature, and date

• Assessment result Must show that the person is competent and authorised to operate

NOTE:

• If Site Authorisation, the RCC Assessment or CTO Assessment section must be completed

• If Theory Assessment, when RCC is completed, the authorised leader is required to verify with a signature that the applicant is authorised to operate

• The learning and development section of the form does not need to be completed

• Issue date = Date of training completion

• Expiry date = 2 years from issue date
