• First and last name on evidence must match the applicant's name in Onsite

• Certificate / card must state Infrared Thermography Level 2 accreditation or certification

• Issuer information must be reflected on the evidence

Issue date = as shown on the certificate / card

Expiry date = 5 years from issue date

• Applicants name to be shown on documentation

• Applicants name must match (However shortened versions such as 'Chris' for Christopher can be accepted)

• Evidence must be issued by an Australian Based Recognized Training Institution

• Actual copy of certificate / degree is required. Letters are not accepted in lieu of this.

• Completion date or issue date must be displayed

• Course title and associated level to be listed on certificate to match the nominated coding and title within Onsite

• Document / Certificate / Number

• Other accepted qualifications (With Major in Chemistry) are Forensics Science, Biotechnology, Nanotechnology and Pharmaceutical

NOTE: Workers must upload an actual degree issued by a university or college demonstrating exactly which course has been completed
