• Applicants name shown on licence must match the applicants registered name in Onsite
• (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)
• If the licence only has the first initial of the workers first name, the licence is acceptable
• Radiation Users Licence to be listed
• Must be issued by a State Radiological Council or State Environment Protection Authority (EPA)
• Black and white or colour copies of this document is accepted
• Must be clear and legible 
• Radiation licence renewal receipt can be accepted as an interim form of evidence for a period of 8 weeks from renewal date providing the following information is displayed in the screenshot
  • Applicants first and last name
  • Licence Number
  • Date of renewal

• Date of completion to be recorded as issue date listed on licence
• Expiry date to be recorded as expiry date listed on licence 

Renewal receipts 8 weeks expiry from date of the renewal

• Applicants name shown on evidence must match the applicants registered name in Onsite
• (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)
• Black and white or colour copies of this document is accepted
• Must be clear and legible
• Evidence accepted from either of the below companies:
  • RADHAZ (must contain code AWR03)
  • Total Radiation Solutions (must contain code RFA02)
  • RF Awareness (must contain reference to ARPANSA Std RPS S-1 at the bottom of the document)
• Certificate must contain:
  • RF EME Awareness Course/Training

• Date of completion to be recorded as issue date listed on evidence
• Expiry date to be recorded 3 years from completion date
