Health & Safety.Form.Journey Management Form	Name to match person registered in onsite Authorisation section must fully completed if the distance to be travelled is more than 100km. If the distance travelled is less than 100km then no authorisations are required Note: After the initial form lodged at induction expires this competency and whether another is needed will be managed at site – company to contact site	Issue date – date signed by manager Expiry date – 12 months from issue date
Safety.Course.MDG25 Awareness Training	Name on certificate to match the person registered TRG Hot Work at Mines and Petroleum Sites to be listed MDG25 Awareness Training can be accepted Must be completed through 3D Safety, or approved training provider Black and white or colour is acceptable	Issue date – date signed by manager Expiry date – 3 years from issue date
Health & Safety.Appointment.Contractor Owner	Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted) WHC-FRM-CONTRACTOR OWNER APPOINTMENT to be uploaded All fields must be completed The company name should match the registered company name in Onsite Must have signature of at least one authorised site approver. Click here for the list of authorised approvers per site	Issue date – to be recorded as the (earliest) date it was signed by the Mine Operator or delegate Expiry date – 3 years from issue date
