COAL MINE WORKERS HEALTH SCHEME ASSESSMENT (COAL BOARD MEDICAL)Medical – (QLD) – For QLD operations only
  • Inductee name and date of birth to be clearly shown on the medical.
  • Must be stamped and signed by the medical practitioner.
  • Must be a QLD Section 4 certificate.
  • Only instance you would require a Restrictive Management Plan for vision or hearing is if the Section 4 states there needs to be a practical test performed on site for colour vision or for hearing to ensure the person can hear sirens/alarms etc
  • Employer listed on Medical is not required to match employer listed in onsite
  • Issue date of medical completion
  • All medicals must be completed within the previous 5 years (UNLESS there is an earlier subsequent assessment review date / recommended date of next health assessment listed on medical (which would then be the expiry date)
  • Chest X-ray field must be completed (a tick in yes, no or date of last chest x-ray completed)

NSW Legislative Health Assessment (Order 45 or Order 43)









New inductees require:

  • NSW Legislative Health Assessment
      • Current Order 45 Health Assessment (HA)
        • An original Order 45 Health Assessment Certificate
        • An Order 45 Health Assessment Review Certificates with the original Order 45 Health Assessment Certificate; OR
        • An Order 45 Health Assessment Review with the original Order 43 Medical

OR

      • Current Order 43 Medical
        • Refers to valid Preplacement, Periodic, and Site Access Medical
        • Order 43 Preplacement or Periodic Medical Assessment Review Reports must be submitted with the original Order 43 Medical Assessment
  • Additional requirements (see Role Requirements):
      • Drug and Alcohol Screening (DAS) test must have a negative result or be consistent with declared medication as confirmed by laboratory testing
      • Functional Capacity Evaluation (FCE) as per role requirements (completed within the last 12 months) 
      • Approved chest imaging:
          • ILO Chest X-Ray, or
          • High resolution CT (HRCT), or
          • ILO B-Read  

Continuing workforce (already inducted) require: 

  • NSW legislative Health assessment, as written above (new DAS and FCE not required – where possible include original completed FCE and DAS in the medical upload so all documents are in a single file) 
  • Chest imaging as detailed above

General Rules:

  • All medicals must be completed within the previous 3 years (all pages must be included)
  • Name must match the registered person 
  • Date of Birth on must match the registered person 
  • Employer Name
    • Order 43: Employer must be listed, matching the company name in Pegasus, (if this is not the current employer, the Order 43 New Employer Registration’ notification from CS Health must be uploaded)  
    • Order 45: Employer is not required to be listed 
  • Must have either Doctor’s name, signature (and AMP number) or Registered Nurse’s name, signature (and ARN number) 
  • Electronic signature accepted 
  • Must be completed by an approved medical practitioner on the list here
  • Where an early review is required (see Early Reviews/ Ongoing Monitoring General Rules)
  • For medicals indicating a restriction or remediation measure (see WHC-FRM-Health Management Plan (HMP) General Rules)

Additional Order 45 General Rules:

  • The Risk Category on the Order 45 Health Assessment must align with WHC identified risk categories (refer to role requirements). Where risk category is uncertain, Risk Category A is accepted.
  • The Coal Mine Worker must be able (‘Yes’ ticked) to ‘commence or continue to carry out the work’ as per image below: 

Early Reviews/ Ongoing Monitoring General Rules:

  • Ongoing monitoring is indicated by an Amber medical result on an Order 43 or Order 45 Health Assessment (as shown on traffic light page) or where written that follow up is required (e.g. 12-month medical review)
  • The latest Order 43 Review Report or Order 45 HA Certificate Review must be uploaded with the original Order 43 / Order 45 HA certificate
  • Order 45 HA Certificate Review can be accepted with original, corresponding Order 43 Medical Assessment Report
  • Where only an early review/ ongoing monitoring only is prescribed (no restrictions/ remedial measures apply) a Health Management Plan (HMP) is not required and does NOT require WHC review

WHC-FRM-Health Management Plan (HMP) General Rules:

  • A HMP must be uploaded together with the most recent Order 43 report/ Order 45 Certificate and additional requirements under the Medical.Assessment.NSW Coal Compliant Medical when a restriction is identified on the medical
  • The latest Order 43 Review Report or Order 45 HA Certificate Review must be uploaded with the original Order 43 / Order 45 HA certificate
  • Order 45 HA Certificate Review can be accepted with original, corresponding Order 43 Medical Assessment Report
  • When the medical has an amber result, and nominates that a restriction applies to some aspect of their normal role – the HMP (check box is ticked indicating restrictions) will require sign off by WHC Representative and upload into the portal
  • Prescription / Corrective Lenses
    • Where the only restriction listed indicates wearing of prescription/ corrective lenses, this does not require an HMP
  • Dust Restriction
    • Where dust restrictions apply, a passed mask fit test to AS1715 (within the past 12 months) is also required to be provided with the medical
  • Hearing Aids
    • Where the Order 45 Health Assessment indicates that a Coal Mine Worker is required to wear hearing aids (Yes box is ticked), then a HMP is required with WHC sign off
  • Weight Restriction
    • Where a weight restriction is identified, any remedial measure exceeding these limits requires WHC sign‑off on the HMP
      • 130kg at Narrabri, Maules Creek or Biodiversity Offsets
      • 125kg for a Gunnedah Open Cut sites (Vickery, Tarrawonga, Gunnedah CHPP).
      • 125kg for HMPs indicating ‘all sites’
    • Any remedial measure below the above restrictions does not require WHC sign‑off on the HMP

  • Where the Order 43 / Order 45 Health Assessment or Review identifies restrictions or Employer Actions, the associated HMPs are to be sent to medicals@whitehavencoal.com.au for review and approval (as per authorised list below)
    • The HMP must be submitted to WHC Medicals with the following items:
      • Full Order 43 Medical / Order 45 Health Assessment (including any updated review reports) 
      • Current approved Chest imaging
      • FCE as per requirements
      • Drug and Alcohol

NOTE:  For new inductees the FCE & DAS must have been completed within 12 months. For continuing workforce (already inducted), please submit original accepted FCE & DAS

Download and complete WHC-FRM-Health Management Plan (from the Whitehaven Coal Document Portal – login details as per this reference guide), ensuring that it has been discussed with the worker, they understand the follow up requirements and they have signed where relevant

  • Worker Name: must match the Order 43 / Order 45 assessment
  • Date of Birth: must match the Order 43 / Order 45 assessment.
  • Site: indicate which site/s the worker will be working at 
  • HMP expiry date/date review required: must be entered and, at a minimum be the date per the medical / health assessment for which the review is required. This date can be earlier than the nominated review time

  • Issue Date = Original Order 43 or 45 date of assessment/ date of original preplacement or Periodic (NOT the review date)

NOTE: For Early Medical Review documents, the issue date should be the date of Original Order 45 ‘Date of Health Assessment’ or ‘Last health assessment date’ on the Order 45 Health Assessment Certificate Review.

  • Expiry Date = If the Order 43 or 45 medical does not require an early review, the expiry date is 3 years from calculated issue date (written as ‘Date of Health Assessment’ on 45 Health Assessment certificate)


Where an early review is required

  • Issue Date = Original Order 43 / Order 45 assessment/examination date (NOT the review date) 
  • Expiry Date = The date the early review is due

Example: 

    • Original Order 43 or 45 assessment/examination date was 13/05/2019 
    • Review time indicated is 12 months
    • Expiry date would be 13/05/2020

NOTE: If the expiry date of the latest Health Assessment Certificate Review is greater than 3 years from the Last Health Assessment Date (Original Medical/ HA Date) then expiry date should be 3 years from Last Health Assessment Date


  • Please note that where a ‘date of review’ is not indicated on the certificate, the expiry would be mapped to the date that the Order 43 / 45 doctor has signed off 
  • An expiry date earlier than the nominated review time can be accepted 
  • Where an updated Order 43 / Order 45 Review Certificate indicates no ongoing review requirements (routine review only), the date needs to be updated to the original medical expiry date (E.g. 3 years from Date of Health Assessment indicated on Order 45 HA Certificate)
  • Where a contract partner has uploaded an older version of the HMP document (WHC-FRM-Early Review Plan), this can be accepted

  • For Workers with either the All Open Cut – Bulk Fuel Lubricants Delivery Driver (MR, HR, HC, & MC) OR the Narrabri Surface –  Delivery Driver (MR, HR, HC & MC) selected on the ATUI form, are not required to have completed an Order 43 Medical / Order 45 Health Assessment
  • Minimum Medical Competency Requirement is RMS Fitness to Drive Medical (expiry in Pegasus as stipulated in the RMS medical) 
  • Where an Order 43 Medical / Order 45 Health Assessment has been provided it will be accepted & there is no requirement to provide RMS Fitness to Drive Medical in addition 
  • Medical Competency for Delivery Driver Role are to be uploaded under Group Competency – Administration.Medical.Bulk Fuel and Lubricants Delivery Driver

Additional requirements for NEW INDUCTEES only.

New inductees working in a coal mining area MAY (depending on their role[s] selection) also be required to undergo a functional capacity evaluation (FCE) as per the below criteria prior to commencing with WHC.

  • Name and date of birth to match the person
  • Must be completed within the past 12 months
  • Must include the Position/Role
  • FCEs can be completed by any registered Physiotherapist, Exercise Physiologist or Occupational Therapist (details must be included)

NOTE: If the FCE indicates an Amber result the FCE report must be approved by WHC. WHC no longer requires a 3-month repeat of FCE (as indicated on old template) and will be assessed at the time of submission.

Medical.-.Chest X-Ray - ILO Report



  • Must indicate that it is an ILO Chest X-ray or Chest imaging report (must be full report)
  • If the Chest X-ray report does not read ‘No evidence of pneumoconiosis, OR ILO Classification 0/0 or 0/1’ the contractor is required to send the report to CS Health to verify there are no concerns that require follow up
  • If the Chest X-ray report recommends a HRCT scan, the HRCT report must also be attached
  • HRCT scans will be accepted
  • B-Read and HRCT are accepted if the date of imaging matches the Order 43 / 45 certificate (refer to Last Chest Imaging Date on certificate). If the date does not match, the report will require WHC verification
