• Applicants name to be displayed on uploaded evidence and to match applicants registered name (however shortened versions of names such as 'Chris' for 'Christopher' is accepted)
• Evidence Documents must capture under Apprenticeship or Traineeship  
• Duration as shown on Documents
• Black and white or colour is acceptable

• Issue date to be recorded
• Expiry date to be recorded

• Contracting company’s letterhead - must be the inducting company Address and Contact Information
• Must state the name of the employee
• Employee occupation
• Details of the work the employee is authorised to perform at site  
• Details of the experience in task to be performed ie. approx years
• Must be signed by a senior manager/supervisor or HR of the contracting company                                                                              
• Must include the title of the person signing

Note: Company Directors or Managers are the only individuals that can sign off their own Statement of Competency

• Issue Date = date on the letter (must be dated within 3 months at the time of submission
• Expiry Date = no expiry date to be recorded
