• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

• Accepted types of certificates are:

  • Certificate of Completion

  • Statement of Attainment

  • Statement of Completion

• Must show completion of either:

  • CPCCDE3014 Remove non-friable asbestos

  • CPCCDE3015 Remove friable asbestos

  • CPCCDE4008 Supervise asbestos removal

NOTE: If an asbestos removal licence in a business name is submitted, the worker must be listed as one of the nominated supervisors overseeing the relevant class of asbestos removal work

• The Issue date in Onsite should be recorded as the date of completion shown on the certificate / card
• Expiry date to be listed as shown on the evidence
