• Name on card to match the person registered in the system
• Card must include test date and Pass result of 'Y' and the Assessors name
• Card or attendance list/sheet to be issued from RedOHMS Group
• Black and white or colour is acceptable
• Scanned document to be clearly legible

Exemption Process:

An exemption may be granted for a training competency requirement listed in the Training Matrices for an employee or contractor. Exemptions shall be applied only after careful consideration and risk assessment. Please click here for the template and business rules.

• Test Date to be entered as training completion date
• Expiry to be entered as 1 year from completion date

• Name on card or certificate to match the person registered/in the system
• Card must include test date and Pass result of 'Y' and the Assessors name.
• Card or attendance list/sheet to be issued from RedOHMS Group
• Black and white or colour is acceptable
• Scanned document to be clearly legible

• Test Date to be entered as training completion date
• Expiry to be entered as 1 year from completion date

• Name on certificate to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
• Document must be a Statement of Attainment
• Statement of Attainment must indicate either:
  • Drug and Alcohol Testing
  • Drug and Alcohol Screening
  • Approved Drug Screening Officer
  • Collect specimens for drug of abuse testing
  • Collect pathology specimens other than blood
  • Collect pathology specimens other than blood for specialized testing

• Competency unit/qualification code must match exactly
• Must be issued by an RTO with Nationally Recognised Training logo
• Black and white or colour is acceptable
• Scanned document to be clearly legible

• Issue Date to be entered as date of training completion
• No expiry

• Name on card to match the person registered in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
• Certificate of Completion to show completion of Audiometric Hearing Test
• Completion date or issue date must be displayed
• Black and white or colour is acceptable
• Scanned document to be clearly legible
• Issue date to be entered as date of test completion
• Certificate to be issued from Yara H&S Department

• Issue Date to be entered as date of training completion
• Expiry to be entered as 2 years from date of issue
