This competency may be completed after commencement of employment but is required PRIOR to becoming an Isolation Officer.

• Applicant's signature and signed date
• Authorised Assessor's complete name and signature and date
• Must show that the person is deemed competent

Exemption Process:

An exemption may be granted for a training competency requirement listed in the Training Matrices for an employee or contractor. Exemptions shall be applied only after careful consideration and risk assessment. Please click here for the template and business rules.

Issue date = Use the date of assessment

Expiry date = 2 years from issue date

Note: It is acceptable to submit either TAN or Ammonia Asssessment

• Must show that the person is deemed competent for ALL fields

• Trainee’s Details to match trainee’s profile.

Note: Shortened versions of first names are accepted, such as: "Chris" instead of "Christopher" & "Wil" instead of "William“

• Form must be completed with name of individual and signed off by both the Trainee and Isolation Officer

• Issue date = The most recent sign off date on the form.

•  Document submitted must the Trainee Isolation Officer Log form
• Trainee’s Details to match trainee’s profile.

• Form must be completed with name of individual and signed off by both the Trainee and Isolation Officer
