Site.Course.Class 1 Aviation Medical Cert

• Full name must be printed on certificate
• Issued by Australian Government Civil Aviation Safety Authority
• Class 1 coverage printed on certificate
• Issue/Print date on certificate

Issue Date: As per attachment

Expiry Date: 1 year from issue date.

Site.Licence.Pilots License

• Full name must be printed on digital licence
• CPL- H must be printed on digital license
• Issued by Australian Government Civil Aviation Safety Authority
• CPL doesn’t expire and is indefinite.

Issue Date: Not required

Expiry Date: Not required

Site.Licence.LLH Rating

• LLH rating must be printed on CPL passport paper
• Issue date must be printed
• Name printed i.e B.J.TEST next to ARN number on top right side per attachment example

Site.Course.CRM or TEM Training

• All aircrew (including AusNet Services personnel nominated on the flight manifest as crew) flying for AusNet Services should have undergone Crew Resource Management (CRM) or Threat and Error Management training at intervals not exceeding two years. In the case of single pilot operated aircraft, completion of Aircrew Decision Making (ADM) course or a suitable Threat and Error Management course will be acceptable.

Issue date: Issue date per the training issued date.

Expiry date: 2 years from issue date
