Radiation.Statement of Attainment.ED182 - Radiation Safety National Module	Name on Certificate must match the person registered (Shortened versions such as 'Chris' for Christopher can be accepted) RTO Name, National Provider Code / RTO Number & Logo to be displayed on document Must be a Statement of Attainment that lists 'ED182 - Radiation Safety National Module' Issue or Completion date must be listed on document Licence or Card also accepted if all requirements are met, front and back of card must be supplied Colour Copy or black and white accepted	Issue Date: to be recorded as shown on the evidence Expiry Date: 2 years after completion date

Radiation.Training.Radiation Safety Certificate	Name on Certificate must match the person registered (Shortened versions such as 'Chris' for Christopher can be accepted) Certificate to list Radiation Safety	Expiry Date: None
Training. Certification. Level 2 Non-Destructive Testing	Evidence can be provided if the worker has previously completed the training. Certificate must be as per example provided. Evidence must show correct full name as registered in the system Certificate is issued by AINDT, following details: Certification details Industrial Sector Level Registration Number Issue Date Expiry Date Issue Number Endorsement With AINDT seal Signed off by the President and Chairman of the Board of AINDT QR code included to validate certificate issued.	Issue Date – Record issue date as shown on the document Expiry Date: every 5 years.
Training. Certification. Radiography Assistant Training	Evidence can be provided if the worker has previously completed the training. Certificate must be as per example provided. Evidence must show correct full name as registered in the system Licence issued by Queensland Health with QLD Government Logo visible Licence Number Licence issued as per the requirement of Radiation Safety Act of 1999 Following details must be shown in the document: Radiation source (must include but not limited to in the sample) Radiation practice listed (must include, but not limited to in the sample provided) Conditions Where condition is marked, details of the condition is also listed. Certificate/Licence by the Chief Executive or Delegate of the Queensland Health Decision Date listed Expiry Date must be current	Issue Date – Record issue date as shown on the document Expiry Date: every 3 years.
