  • Evidence can be provided if the worker has previously completed the training.
  • Certificate must be as per example provided.
    • Evidence must show correct full name as registered in the system
    • Certificate is issued by AINDT, following details:
      • Certification details
      • Industrial Sector
      • Level
      • Registration Number
      • Issue Date
      • Expiry Date
      • Issue Number
      • Endorsement
    • With AINDT seal
    • Signed off by the President and Chairman of the Board of AINDT
    • QR code included to validate certificate issued.

    • Licence Number
    • Licence issued as per the requirement of Radiation Safety Act of 1999
    • Following details must be shown in the document:
      • Radiation source (must include but not limited to in the sample)
      • Radiation practice listed (must include, but not limited to in the sample provided)
      • Conditions
      • Where condition is marked, details of the condition is also listed.
    • Certificate/Licence by the Chief Executive or Delegate of the Queensland Health
    • Decision Date listed
    • Expiry Date must be current
