| Medical.Assessment.Industry medical | - Worker role not involved in the process or extraction of coal or otherwise engaged in the mining process may provide a pre-employment or fitness for work medical conducted by an Australian medical practitioner.
- Medicals must correctly reflect the worker's personnel details.
Example: Rail industry medical, Transport worker medical, Generic pre-employment medical from an allied health services provider such as Sonic Health / Humanomics. IF AMBER or RED PROCEDURE: Medical is marked "Amber" or "Red" - Employer Action Section: “Is the Employer required to take any action as part of the follow up?”
- Marked "YES" = Will need to supply a Medical Management Plan (MMP) along with Medical Assessment Order 45 Medical Form
- Marked "NO" = Will only need to supply Medical Assessment Order 43 Medical Form
If a Medical Management Plan is required it must: - Be presented on a company letterhead
- The medical has been approved by a senior company representative and must be signed by a senior company representative, e.g. supervisor, HR representative, return to work coordinator and also the worker must sign.
- The employee is fit for duty to perform tasks whilst on the Malabar site under the medical restriction.
- MMP must address the recommendations from the Doctor
- State that the employee’s medical has been approved by the company
- Be dated within the last 3 months
- Must be uploaded as a PDF
NOTE: Medical Management Plan is not required for the restriction of wearing prescription glasses. | Issue Date = Date of examination Expiry Date = 3 years (unless O43/O45 medical stated earlier follow up review is required, in which case the subsequent assessment date noted on the medical must be recorded)

| Medical.Assessment.Order 43 Medical Form OR Medical.Assessment.Site Access | - All workers must provide a Coal Services Health - Health Assessment Certificate compliant to the requirements of Order-45.
- If the provided document is an Order-45 Health Assessment Certificate it must display:
- Position specified within the medical must be equivalent to the Role within Pegasus. Where differences occur, these are to be verified with site.
- Risk Category
- Similar Exposure Group
- Health Determination
- Any recommendations or restrictions.
- If the provided document is an Order 43 Periodic Medical, it will be accepted provided that:
- It is a Periodic Medical with Date Of Assessment no older than 3-years.
- It is conducted by Coal Services Health approved medical practitioner and approved by Coal Services Health.
- The worker name matches the employee name.
- If the provided document is an Order 43 Pre-Placement Medical, it will be accepted provided that:
- By uploading the medical the Employer confirms the document is the workers FIRST Pre-Placement Medical upon commencement in the NSW Coal Mining Industry.
- It is a Pre-Placement Medical with Date Of Assessment no older than 3-years.
- It is conducted by Coal Services Health approved medical practitioner and approved by Coal Services Health.
- The worker name matches the employee name.
- If the provided document is a written exemption from a Malabar approved authoriser, it will be accepted in lieu of a medical, with the expiry date set as advised.
Approved Authorisers:- Health & Safety Superintendent (Mark O’Brien)
- Health Coordinator (Michelle Johnson)
- Health and Safety Administrator (Crystal Lacey)
- Must be completed by an approved medical practitioner on the list here
- We will accept:
- The full medical report; OR
- An extract of assessment report containing patient details, assessment date, Review date, doctor’s determination and restrictions or recommendations.
