• Issue Date: date of examination by the EMO (EMO date not the NMA sign off date)
• Expiry Date: two (2) years from the date of examination UNLESS there is a review date which then becomes the end date

• Applicant name on medical to match the name registered in the system
• Applicant date of birth (DOB) must be displayed on medical and match the DOB in Onsite
• Must be stamped and signed by the medical practitioner
• Must be a QLD Section 4 or health assessment provided by Resources Safety & Health (ResHealth)
• If the fitness for duty section is ticked - “Is fit to undertake the proposed/current position subject to the following restriction(s)” OR “Is not fit to undertake the proposed/current position because of the following restriction(s)” -  Do not process as the medical needs to include a Management of Current Medical Restrictions Form and be uploaded under the restricted medical competency. 
• If medical is uploaded as Medical.Certificate.QLD - Coal Board Medical - Surface - RESTRICTIONS the role selected must be a surface role.

No client Escalation necessary providing the restricted medical meets the BR requirements including that it is accompanied by MMP which is signed off by authorised approver. This is accepted. If it does not meet these requirements it can be returned.

• Must supply a Drug & Alcohol Test 

• Date of test must not be older than 30 days as at date of induction

• The new health assessment provided by ResHealth is the equivalent to the paper-based Section 4 and will become mandatory from 1 April 2023. This can be accepted before the 1st of April.
