• Applicants name and DOB to be displayed on uploaded evidence and to match the applicant's registered details
• "Sojitz Underground Crinum Contracts Manager Approval to Induct" section to be filled out, approved and signed
• Must be clear and legible
• Black and white or colour copy of the document is accepted
• Sojitz Crinum underground Contractor Approval to Induct form can be accepted

• Issue Date = Date of Sojitz Underground approval ("Sojitz Underground Crinum Contracts Manager Approval to Induct" section)
• Expiry Date = No Expiry is required

• Applicants name and DOB to be displayed on uploaded evidence and to match the applicant's registered details
• Approval to Induct form to be filled out, approved and signed
• Must show evidence of approval to complete the Gregory Site Induction
• Must be clear and legible
• Black and white or colour copy of the document is accepted

• Issue Date = Date of approval (Sojitz Gregory Mining /Authorisation to Induct)
• Expiry Date = No Expiry is required
