• Name on certificate to match the person registered
• Electrical Apprentice Indenture papers / Training Contract / Trainee Licence to be supplied
• Must be issued by the state based TAFE or Apprenticeship Network Provider
• Black and white of colour is acceptable

• Issue date to be recorded (Commencement date of Apprenticeship/Traineeship)
• Expiry Date to be recorded (Nominal Term of Training Contract)
• Signature and Date in Apprentice Network Provider Signature section of Training Contract

• Name on Licence to match name of applicant in Onsite
• Licence / registration must be current
• Registration card must be endorsed as a “lift cabler” or an “open cabler”
• Must be issued by an ACMA accredited Industry Register
• Both sides of the Licence are required to be shown within the upload
• Black and white or Colour copy can be accepted

• Issue date to be recorded if displayed on licence

• Expiry date to be recorded as shown on the licence

Name on Electrical Licence to match the person registered

Accepted form/s of evidence:

• Electrical contractor licence
• Supervisor certificate licence showing 'Q' can be accepted
  
• New Zealand Electrical licence listing 'Inspector'
• New Zealand practicing licence listing 'Electrician'
• Screen shot showing that the licence is current for an interim period of 4 weeks and then the licence needs to be supplied
• Digital cards/certificates are accepted

Must be issued by nominated / relevant government department based on state of issue and reflect the corresponding logo.

Black and white or colour is acceptable

• Expiry date to be recorded as shown on licence
