  • Name on Licence must match registered name in Onsite
  • Refrigeration and Air-conditioning to be listed
  • Must be issued by relevant state government authority
  • Duration as per licence
  • Relevant state government authority logo must be present
  • Black and white or colour is acceptable

New Zealand Licence will be accepted in an individuals name 

  • Issue and expiry to be recorded as shown on licence

  • If a Sole Trader the name on Licence to match the person registered
  • If a Company, the name on Licence to match the company registered

  • Black and white or colour is acceptable

Otis Sub Contractor Agreement – State Restrictions

  • Otis Subcontractor Agreement Template to be used
  • The Company Name and ABN - Needs to match the company name as registered in Pegasus. If it doesn't match, government certified evidence must be provided to link the business names together. This is ideally a letter or download from ABR (Australian Business Register) showing the linkage.
  • All fields on the form are to be complete
  • Form to be signed and/or dated by company representative
  • Form to be signed and/or dated by Authorised Otis Representative
  • Any form clauses change must be approved by Otis Managers
  • Subcontractor agreement must contain the complete pages as per the templates provided - Document will never be 1 page only


Please Note: Pages 10, 11 & 12 are optional clauses and may be left BLANK

  • Record the date signed as issue/completion date within Onsite
  • Document has different expiry date. Please check the agreement clause.

  • When no expiry is shown refer to the termination clause 10.1
